Abstract
Puskesmas as one of the public service agencies requires the existence of an information system that is accurate and reliable, and adequate to improve services for patients and related environments. With a wide range of services, of course, there are many complex problems that occur in the service process at the puskesmas. The medical record information system is a system that aims to manage patient data who register for treatment which is useful for preventing procedural errors in the implementation of registration and data management. The method used is the waterfall method, namely the stages of requirement analysis and definition, system and software design, implementation and unit testing, integration and system testing. This study used a qualitative approach, where the process of collecting and collecting data was carried out by interview and observation. PHP programming language with phpmyadmin database and using a sublime text editor. The results obtained are the application for UGD registration at Public Health Center of Pakusari Jember.
