Abstract

Case of missing and misplaced medical record files (missfiles) occurs because no medical record files are found in the storage unit when the medical record file will be shown from the service unit, it happens because the officer takes out the medical record file from the lending unit without being recorded in the storage book. The result of study at the Nangkaan Health Center in Bondowoso District still found /missfile/ in the medical record file, From the result of random sampling which have been done by researchers found that there were 7 files which are wrong location and 108 files which are missing. The purpose of this study is to analyze the priority factors which caused the /Missfile/ Patient Medical Record File in Nangkaan Health Center, Bondowoso Regency. This type of research is qualitative, Data collection technique by observation, interview, and brainstorming. The method in this research uses Multiple Criteria Utility Assessment (MCUA) which the subject is Medical Record officer. Data analysis uses 5M management elements (Man, Money, Method, Material, Machine). The results showed that the priority of the occurrence of missfile based on Man, it is because of medical record officer education and training. Money, there was no budget funds for tracer and training, Method, there was no standard operating procedure (SOP) to take medical record file, Material, for using a medical record folder, which is only part of the file that uses a folder and another is not, and Machine, expedition book and tracer. The priority results of problems with MCUA, is the use of expedition books that have not been maximized, during this time, officers often do not record the medical record files that come out in the expedition book.
