Abstract
Completeness of filling out the informed consent must be 100%. The number of incompleteness informed consent forms that occurred at Baladhika Husada Jember Hospital from January to June 2024 with an average of 72.73% and a completeness level of 27.27%. This study aims to analyze the factors causing incompleteness filling out of the informed consent form for general surgery inpatients at Baladhika Husada Jember Hospital based on Robbins' performance theory, namely the ability, motivation, opportunity factors. This type of research is qualitative and data collection techniques using interviews, observations, documentation, problem priorities using scoring techniques and improvement efforts using brainstorming. The subjects of the study were the head of medical records, general surgeons, inpatient nurses, heads of inpatient rooms and medical records officers. The object of the study was the incompleteness of the informed consent form for general surgery inpatients. The results of the study which became the factors causing incompleteness filling out of inpatient medical records based on the Ability factor were the lack of knowledge possessed by doctors and nurses regarding the standard for completeness of filling out the informed consent form. Motivation factor is the absence of reward given to doctors and nurses. Opportunity factor is the absence of SOP and job description that is still not specific. Recommendations for improvement efforts that can be proposed are creating SOP for filling out informed consent, providing socialization to doctors and nurses regarding informed consent filling standards, improving job descriptions by adding descriptions of main tasks related to filling out informed consent, and implementing rewards. The conclusion of this study is that the factors causing the problem are in the variables ability, motivation and opportunity. Key words: Informed Consent, Incompleteness, Inpatient, Hospital.
