Abstract

The completeness of filling medical record at Mitra Medika Bondowoso Hospital in 2017 was 60%, where the standards applied was 100% complete, with a minimum value of 80% according to the level and criteria for passing hospital accreditation based on the National Standards for Hospital Accreditation (SNARS) in 2018. Based on this matter, this study aimed to apply the Six Sigma method in improving the quality of completeness in filling medical record at Mitra Medika Bondowoso Hospital. During the Define stage, 699 out of 3450 items or 220 out of 230 medical records used have not been completely filled from January to March 2018, based on quantitative analysis including items in identification, reporting, recording, and authentication. This made the sigma value of completing the medical record which was calculated in the Measure stage was at the value of 3.71 Sigma. The caused of incompleteness problems were identified in the Analyze stage using causal diagrams and 5M theory, based on observation, interviews, brainstorming and documentation. The results of this study were obtained after formulating several alternative solutions in the Improve phase, which were then carried out and documented to be disseminated in the Control stage, there were making and implementing the Standard Operating Procedures of filling the medical records, socializing the Standard Operating Procedures of filling the medical records and adding more medical recorders. Some suggestions that could be given are periodic evaluation of the implementation in filling the medical record activities and socialization activities, developing the human resources, and revising the medical record forms used.
