Abstract

Patient who comes to a health-care will be given a medical record number functions as patient’s identity. The problem at RSU Wonolangan Probolinggo are the files over loaded, the lack of thoroughness officer in searching for past history files and that files were can not found were made new document files, causing duplication medical record number.The purpose of this research is to analyze the priority factor as the cause of duplication outpatients’ medical history number at RSU Wonolangan Probolinggo. This research was qualitative which used interview to collect data, observation, questionnaire, documentation and USG method. The result of this research showed that the duplication of medical record happened caused by the staffs’ education which is not qualified with diploma of medical record, the ignorance from staffs about giving number, the undetail SOP numbering and no socialization about SOP numbering, the file folder not based on standard yet, and there is no warning system if SIMRS was double. The priority cause of this problem was found used USG method that were undetails SOP numbering and no socialization about 52 score. Thus the researcher gives suggestion to revise and socialize SOP numbering. Other solution is an evaluation about socialization of SOP numbering, recommendation about staffs’ next education, giving training about medical record to staffs, and renewed system of SIMRS to minimize the duplication of medical record numbering.
