Abstract
The implementation of medical records is a factor to determine whether the health-care at the hospital is good or not. The problem at Mitra Sehat Situbondo hospital are inadequate filling space, there are many accumulated medical record documents, and that files were can not found were made new document, causing duplication medical record number. The purpose of this research is to analyze the priority factor as the cause of duplication medical record number at Mitra Sehat Situbondo Hospital. This research is qualitative wich used interview data, observation, documentation, questionnaire and CARL method. From the result of this research found some problems there were no reward and incentive that should be given to officers, registration officers and filling officers never joined medical record training, the officers’ ignorance about SOP. The priority cause was found used CARL method that where the officers’ ignorance about SOP at Mitra Sehat Situbondo Hospital about 250 score. The researcher gives suggestion from the priority causes the problems that the hospital should implement socialization for existed SOP and give monitoring SOP implementation every 6 months once to minimize the duplication of medical records.
