Abstract

A complete medical record can be used as a health service administration data record to support quality assurance information, while an incomplete medical record will hinder the provision of information. Based on the results of a literature review study of inpatient medical records at several health centers, the completeness rate was less than the completeness standard, namely 100%. The purpose of this study to analyze the factors causing the incomplete filling of inpatient medical records at the Public Health Center. This type of research is a literature review with 11 journals selected according to the inclusion criteria. The following factors cause incompleteness of filling in inpatient medical records, namely the absence of Standart Operational Procedure (SOP) related to filling in medical records and socializing Standart Operational Procedure (SOP), monitoring of medical record files, busy doctors, absence of training related to filling medical records, lack of compliance of health workers related to filling medical record files, Doctor's working period, Lack of knowledge of officers related to filling out medical records, High workload of officers, Lack of rewards and punishments related to filling in medical records and incomplete formular designs. Suggestions in this study are implementing reward and punishment policies, conducting monitoring activities about medical records, socializing Standart Operational Procedure (SOP), and holding training about medical records.
