Abstract

From the survey results in dr. Moh.Saleh Kota Probolinggo researchers found the problem on the completeness of an informed consent form on file medical records of patients given medical action. The process of writing an informed consent form is still done manually, so many forms that are not filled. It can affect quality of medical record file but it also can make it difficult for the clerk if the informed consent form scattered. It is expected that with the informed consent form electronic program can simplify the process of filling the form in the informed consent form so that no form is not filled. The purpose of this study was to design an informed consent form electronically in dr. Moh. Saleh Kota Probolinggo. This study uses a waterfall with a qualitative research design and data collection techniques by researchers is the observation, in-depth interviews, documentation and discussion. Coding using notepad ++ applications as well as the PHP programming language for the creation of software and My Sql for database creation. Program informed consent form electronically can be developed again by adding other modules, namely a digital signature.
