Abstract
Informed consent form is one of form in medical record file about patient's consent after doctor being given an explanation. For all high-risk medical measures must have a consent which is written. Based on preliminary survey at Pajarakan Community Health Center Probolinggo, there are seven of the ten informed consent at Community Health Center this are still do not written in commplite. In addition the informed consent form is still not up to standard and that is only one type of informed consent form for all services types. This aims of this reseach is to plan or designing informed consent form and medical records at dental clinic of Pajarakan Community Health Center Probolinggo. The method used qualitative with collection techniques data by observation, interviews and brainstorming. The results of this research is known that the design of the informed consent form used is not in accordance with the standards and user requirements that may lead to incompleteness of filling as well as the absence of informed consent and there is no medical records specifically dental poli. therefore the design of new informed consent form based on 3 aspects those are physical aspect, anatomy aspects and content aspects as standard and user needs. It is advisable to improve the efficiency and effectiveness of entry data should use informed consent and electronic medical dental records.
