Abstract

Medical record document is one part of patient safety whose privacy should be maintained from hazard that include chemical hazard, physic, theft and counterfeit, and tucked away cause misfile. The misfile event is when medical record not finding in the filling room when medical record will be removed from medical record unit. In Maternal and Child Muhammadiyah Probolinggo Hospital found misfile on medical record of 16%. It is caused by manpower factor in restore medical record, method factor is implementation medical rechord loans and returd SOP not optimal, machines factor is the filing rack too far, and mother nature factor is distance between rack too narrow. Therefore, important to do analysis with purpose know the factors that affect the misfile of medical record patient in Maternal and Child Muhammadiyah Probolinggo Hospital. The study method used in this study is fishbone method, the data obtained from interviews, observations, and brainstorming. The result of the study, manpower factor stated the last education medical record officer is SMA and S1 Law, method factor stated that implementation SOP not optimal, machines factor stated that filing rack is too far, and tracer usage not optimal, material factor stated availability of expedition books is no constraint, and mother nature stated that filing room can not save file during 5 years of storage.
