Abstract

Along with technological advances in various fields, all activities or human work to improve the quality of self is done with the development of computer technology is increasing rapidly from day to day. Information technology is one of the technologies created as a human aids to do various tasks so that the tasks done can be more effective, efficient, easy fast and accurate. When the registration of patients can not be separated from the application registration at the Puskesmas. Kendit Situbondo Health Center is less than the maximum in carrying out patient registration due to still using manual system. So still have some problems as when the officer serves the patient is ± 15 minutes, starting from the patient to register until handled by the doctor. Then often the accumulation of patients in the waiting room due to the length of service at the registration counter. The purpose of this research is to make application of outpatient registration at Kendit Situbondo Health Center by using waterfall method which includes analysis and definition of requirement, software system design, unit implementation and testing, system integration and testing, and operation and maintenance. This application has only 1 access that is user. In the application includes patient registration, patient data storage, patient data search, and generate patient visit information per day, per month and per year.
