Abstract

Duplication of medical record files at Kaliwates Hospital of Jember influences the service giving, the doctors can not see past patient history. The occurrence of a lot of files, the using of paper is not reasonable, the waiting time of the old patient due to file searching, the lack of guidance of the officer in searching the previous history file and files that are not found a new documentary file that caused the duplication of medical record number. This study aims to analyze the factors causing duplication of medical record files at public hospitals Kaliwates Jember in 2016. Type of research conducted in this study is using qualitative analysis with cross sectional approach with 4 research subjects consisting of head of medical record and 3 officers registration . In this study the results of the voting or second ranking for the first priority is about SOP that is there are constraints on the patient if not carrying medical card then the officers have trouble finding medical records file, second priority is the age of officers over 45 years feel there is influence in the provision of files and the third priority is that there is a lack of file storage cabinets. To overcome these problems, it is advisable for job placement in storage and file collection are productive officers, it is necessary to add medical record officer with medical record background, officers must give motivation to them selves and fellow the others, it is necessary to socialize SOP to the officers so that officers understand the procedure of filling the good numbering, and also need the addition of storage rack file medical record to support work activity.
