Abstract

Dental medical record forms an important means of communication because it contains detailed data on all the actions performed dentistry medical personnel. Based on the preliminary survey at Candipuro Public Health Center of Lumajang, the dental medical record form is a sheet of blank HVS paper <70 gram, the form has no headings, introductions, instructions, and no close. Only found in the grouping of identity items in the form of name, family, age, address and number of insurance. The purpose of this research is to design dental medical record form at outpatient unit Candipuro Public Health Center of Lumajang. The method used qualitative with action research approach, data collection techniques of observation, interviews and brainstorming, the research subjects are 2 TPPRJ officers, chairman of UKP, dentist and head of Candipuro Public Health Center. Data analysis in this study used qualitative data analysis with action research approach that includes diagnosing (problem diagnosis) lack of old dental medical record form, planning action user requirement and design of new dental medical record form, taking action completeness of form filling, and evaluating action of final dental medical record design form. The result of the research is a new dental medical record design which is in accordance with dental medical record standard and based on user requirement covering physical aspect, aspect of anatomy, and content aspect. Should design of new dental medical records form can be applied in Candipuro Public Health Center to support the completeness of dental medical records information of patients and should be evaluated periodically to find out the level of completeness of information in patient dental records.
