Abstract
Nursing assessment form is one record observations of nurses to patients and help care that has been given to the patient. Based on the initial survey in RSUD dr. Abdoer Rahem Situbondo, less nursing assessment form design according to user needs. Paper thin, incomplete assessment items, the layout is irregular so as to make the clerk lazy and incomplete data. This study aimed to analyze the design of nursing assessment form (RM.06) inpatient medical record documents RSUD dr. Abdoer Rahem Situbondo Year 2015. The research is a qualitative research, collecting data on observations, interviews, and focus group discussions, the informants in the study of 10 people including the head of the nursing room, nurse and head of the medical record. Based on the results of this research is that the design form nursing assessment paper used is not in accordance with the standards and the needs of users that can cause incomplete filling social data and medical data of patients therefore design form nursing assessment newly designed taking into consideration three important aspects namely, the physical aspects , anatomical aspects, and aspects of the content in accordance with the standards and needs of the user. So the need to evaluate nursing assessment form on a regular basis, especially aspects of the content in order to improve the quality of nursing assessment of the patient.
