Abstract
The information system of outpatient private midwife practices is a form of information system that consists of activities of the health services provided in private practice midwives health services consisting of the registration activities, examinations, drug delivery, payment and reporting. In designing this system the researchers used Waterfall method using the Flowchart, Diagram Context, Data Flow Diagram, Entity Relationship Diagram, and in implementing the program using Microsoft Visual FoxPro 9.0. This is made based on the observation of researchers, processing data of patients in Midwife Practice Private still using manual systems which all activities in collecting data and the first time a patient registration is still done manually by writing down in the register book and takes old and less efficient. When patients forget to take control card, the clerk will give you a new card so there is no continuity of data. This has an impact on service activities which lead to the midwife should ask the patient history make a visit earlier time. In the future, for the development systems, researchers suggest using FAST methode, for the development of tests such as the Pap smear, general inspection. Should be done against the development from visiting patients in order to view a list of previous patient visit history whenever treatment
