Abstract

Based on the result of registration officer interview on preliminary survey, it discoverd if the patient which didn’t bring treatment card have to wait for a while to get his/ her files and if it didn’t found, the registrator will make an new file which mean there will occur a data duplicate. Otherwise, the file which is to thick and inactive will be destroyed, so that the files of the patient who had it can’t be seen anymore. In other case if the files broke or lost, there will be more waiting time for the patient and more data duplicate (especially in Mother and Child Health Treatment because there are more patients who forget to bring their treatment card). The purpose of this research is to design and make a web based medical record information system in mother and child treatment of puskesmas Bangsalsari Jember. The result is a web based medical record information system in mother and child treatment with Waterfall method which involve Requirements analysis and definition, System and software design, Implementation and unit testing, Integration and system testing dan Operation and Maintenance. (Sommerville, 2003). This information system is reserved for medical services officer and medical recorder which divided into Administrator, medical services officer and midwife. The function of this information system is including patient social data registration, patient social data searching, patient examination data, patient social and examination data storage and a few output like amount of patient visit with free determined time period and partus estimation.
