Abstract

Informed consent form is one of the forms in a medical record file that containing the patient’s consent given to the doctor after being given an explanation. Written consent is given to all high-risk medical procedures. Based on the preliminary survey in RSUD Genteng Banyuwangi, the highest operative medical procedures in 2015 is caesarean section, reaching 332 medical procedures. Design an informed consent form is not using appropriate paper standards and the absence of grouping data regarding the type of information given to the patient. This study aims to analyze and design of an informed consent caesarean section form in RSUD Genteng Banyuwangi 2016. The method used qualitative and data collection techniques using observation, interviews and brainstorming. Based on the results of this research is that the design of an informed consent form that is used not in accordance with the standards and the needs of users that can cause incomplete charging and hamper the implementation of the provision of informed consent therefore design an informed consent form new ones, designed taking into consideration three important aspects, namely physical aspects , aspects of anatomy and content aspect in accordance with the standards and needs of the user. Form should need to be evaluated periodically to design a new informed consent form in order to suit the needs of users who may change from time to time.
