Abstract
Science and Technology (IPTEK) and the greater public demand for the quality of health services then we are required to provide fast and professional services to the needs of medical information. Borrowing and returning medical record files can not be separated from the medical record system at the Hospital. Maternal and Child Hospital Muhammadiyah Probolinggo less than the maximum in carrying out borrowing and return of medical record files because the system is still manual. So it still has some problems such as frequent occurrence of misplaced medical record files and filling officers have difficulty in monitoring the existence of medical record files that come out and enter from the filling room. The purpose of this research is to make the information system of active medical record control in Maternal and Child Hospital Muhammadiyah Probolinggo by using waterfall method which includes requirement analysis, design, coding, testing and maintenance. This information system has 2 access that is admin and user. Its content functions include borrowing, returns, data storage, searching, and generating late file medical record information in the form of graphs, in addition to generating information on the amount of medical record file borrowing.
