Abstract

Borrowing and reversion information system of medical record is a computerized system that integrate a process of borrowing and reversion entry of medical record that have a purpose to make medical record controlling easy by medical recorder. Borrowing and reversion of medical record must be done in every service of patient’s clinical treatment in health service facility with the amount of transaction which have varieties depend on the health service facility. Because of that, medical record’s control is very vital in the implementation of health service especially in the medical record’s sector. Researcher do the research qualitatively by gathered the data with Conservation and interview in Maesan’s Local Government Clinic, Bondowoso City and designing borrowing and reversion information system of outpatient medical record using waterfall method. This information system use Program Flowchart, Context Diagram, Data Flow Diagram, and Entity Relationship Diagram with implement the result of design into a program use Microsoft Visual Basic 6.0. With collaborating information technology and borrowing and reversion transaction of medical record results an information system that can make an entry of borrowing and reversion of medical record integrated so it can be able to make the medical record control becomes easier, faster, more effective, and more efficient.
