Abstract
Jember Lung Hospital has been applied SIMRS since 2006. Eventhough it applies SIMRS, its information system is still unable to cover processing data record for RL 4 A and RL 4 B. SIRS reporting which related to the morbidity of in-patient and out-patient care.The high number of patients every months effect the manual reporting process duration.The goal of this research is making web-based application of morbidity reporting. The system development method which applied for this research is waterfall methode.Need analysisis done to medical record staff and chief as the interviewers. The application design used Use Case Diagrams, Activity Diagrams, Sequence Diagrams and Class Daigram .The next step was coding which was done by using assembly language of PHP 5 and Mysql database. Examination or testing step uses blackbox methode which is done by medical recor staff as as user. The output of this application is morbidity reporting forn in-patient (RL 4A) and out-patient (RL 4B) which can be downloaded in to excel form. The result of blackbox texting for two users show that the application functionally runs well,easy to acces, and convinent for users . Recomendation for this application is expected to be integrated in the future with SIMRS Jember Lung Hospital.
