Abstract

The form of sign out (resume) of inpatient loaded a clear information to identify the patient can used again when the patient come back to get a treatment on the next visit. Founded the lack of form that causes the information which received is not optimal. The aimed of this research was to design the form of sign out of the inpatient in Bhayangkara Bondowoso Hospital. The technique of collecting data do by interview, observation, and brainstorming. The result of this research is the user want to additional the item on the identify data and clinical data. Based on the physical aspect resume using a NCR paper, rectangular shape with portrait position, white and yellow colors, F4 size (21cm x 33cm), the position of storage rack is portrait. Based on the aspects of the contain of identify data loaded some items, such as the name item, age, gender, pavilion or classes, medical number, date of entry, data of exit , doctor, there is no others diagnosis and action in clinical data. Based on the anatomy aspect heading there is no letterhead and the date of publication. There is no instruction the copy form that sent to anyone else. Each the data items is not separate by the line. In a close part there is no a place or the date of admission filling. The new design of form (resume) using NCR paper that double three paper by addition the item birth, address, telephone number, ICD10 code, action along with ICD9CM, the result of supporting treatment, and the date of control, and the date of admission filling. The suggestion of the researcher the output of the design of the resume form expected to be applied in Bhayangkara Bondowoso Hospital.
