Abstract

The information system of filling is an information system that can help filling officer for transactions of borrowing and repayment in accordance with the medical records that already exist, and can generate reports and tracer . This system uses a system development System Development Live Cycle ( SDLC ) , a design created that database design , Context Diagram ( CD ) , Data Flow Diagrams ( DFD ) , flowchart , Entity Relationship Diagram . The use of software in this stage using Sybase PowerDesigner 12 32 -bit . Through observation , documentation , interviews and focus group discussion in mind that the frequent occurrence of misfile , the recording of borrowing and repayment manual medical record . The results of this research is an information filling system that can help the passage of the filling system
