Abstract

Surveillance of Diarrhea in Jember Public Health Office in recording and data processing not optimal, because it manually. When officer preparing report have trouble, because the officer should recap and find the data that will be need manually. The purpose of this researchs to design system for recording and reporting data on surveillance of diarrheal diseases, for simplify the process of reporting to the Public Health Office. The method is waterfall by analyzing the phases of system requirements, software design, implement, integrate and test. This researchs used qualitative approach, where the process of collecting data by interviewing, observation and FGD. PHP and Macromedia Dreamweaver as a script editor. The results obtained report information system web-based surveillance of diarrheal diseases.
