Abstract

The medical record is the file containing the records and documents about idenditas patient, examination, treatment, action and other services that have been given to patients. The medical record is divided into medical records of outpatient, inpatient, and emergency department. The medical record must also be made in writing, complete and clear or electronically. Record-keeping is the duty of physicians who perform service to patients in health care facilities, including doctors who practice an individual. The process of making medical records private general practitioners in the District Maesan Bondowoso still using manual systems. This research is to use traditional qualitative data collection using interviews, observation, and documentation with the waterfall method. The results of this research is the application of medical record-based fingerprint. For further research is expected to bridging or connecting applications with p.care medical record or primary care ie service information system dedicated to patient status BPJS and need ongoing evaluation on the system by using FAST
