Abstract

Dentistry medical record with odontogram is the most important document of dental medical services. It recorded detailed data regarding the state of the patient and all actions taken by dentistry medical personnel for his patients. In the dental poly at the Jenggawah Public Health Center, they do not use the dental poly form as a medical treatment service tool. The Jenggawah Public Health Center dental poly uses an outpatient medical record form file as a tool for recording patient identity, examination, treatment, actions and other services provided to patients at dental health care facilities. The outpatient medical record form file does not qualify as a dentist medical record form. The purpose of this study is to analyze the design of a dental poly form at Jenggawah Public Health Center Jember. This research is qualitative research. The subjects of this study were the head of the Jenggawah Jember Public Health Center, one dentist and one medical record Jenggawah Public Health Center. at Data collection in this study uses the method of observation, interviews, documentation and brainstorming. The instrument used was a brainstorm sheet. The design of the Jenggawah Jember Public Health Center dental poly form received an evaluation from the dentist and the supervisor before conducting brainstorming with the informants. Based on the results of brainstorming with informants showed that the design of the dental poly form made was acceptable by Public Health Center officer.
