Abstract
A forms and folders must be designed to fulfill users purpose. Increasing the number of patients can be considered a good medical recording and complete so that produce accurate information to support patient care. Mitra Medika Hospital in Bondowoso, the problems identified are incomplete information contained in forms of outpatient, so the quality of forms is still not in accordance with the standard, while in form of outpatient does not use folders. The purpose of this study design forms and folders outpatient medical records. This type of research is descriptive qualitative method of observation, interviews, and focused discussions (brainstorming). Results from this study is the identification of the physical aspects of the used A4 paper, weighing 70 grams, colored white and folders using the Art paper 260 gsm paper yellow for male patients and white for female patients. From the aspect anatomy increase commands form, instruction and introduction, body of form have a limit edge 1,5 cm, while a map enter commands and medical record number. For the contents aspect increase item anamnase, vital sign, nutrition scrinning, history diases patient, histoty allergy, column ICD-X, column ICD-9CM, reconciliation and education and signature doctor and nurse. This results is usefull for outpatient services to be better.
