Abstract

The incompleteness of filling inpatient medical record files at Ambulu Community Health Center from 160 medical record files with a percentage of 29.9% for the incompleteness of filling in the patient's medical record so that it is not in accordance with the standards determined by the Indonesian Ministry of Health (2008) where complete filling of records 100% medical. The purpose of the study was to analyze the incompleteness of filling inpatient medical record files at Ambulu Health Center. Technique of collecting data by interview, observation / checklist, documentation and NGT. The variables used are individuals who are included in individual variables, namely low ability and knowledge. Psychological variables include the attitude of the officer in filling the patient's medical record and motivation where there is no review and punishment. Organizational variables, namely leadership, where leaders never provide oversight of the performance of officers and officers who have multiple jobs because they do not fully carry out tasks according to the design of the work provided. The results obtained are the priority causes of incomplete filling problems, namely the absence of direction and supervision from the leadership. The effort to solve the problem was found that it was necessary to conduct briefings once a week and to be monitored once a month regarding the filling of patients' medical record files.
