Abstract

The filing room is one of the most supportive rooms in the service of medical records, where the patient's medical record documents both inpatient and outpatient care are safely stored because they remember their confidential nature and have legal aspects. At Bhakti Husada General Hospital PT. Rolas Nusantara Medika Krikilan Banyuwangi found missfile events (no medical record documents were found) as many as 320 documents in March 2018. The occurrence of missfile also inhibits the services provided by doctors to patients because there is no information about the history of the previous disease, thus affecting the level of continuity of medical record data in hospitals (Loak, 2013). This study aims to analyze and determine the priority causes of the problem of missfile events using ultrasound (Urgency, Seriousness, Growth) at Bhakti Husada General Hospital PT. Rolas Nusantara Medika Krikilan Banyuwangi. This type of research uses qualitative and data collection with interviews, observation, documentation, questionnaires and brainstorming. The results obtained are the priority causes of the problem of missile events that is not using tracer according to the procedure. Efforts to resolve these problems found that there is a need for socialization regarding the use of tracer in storage shelves and the need for additional human resources in the filing section.
