Abstract
The information system is made based on the problem of organizing the recording of borrowing and reversion medical record file. This information system is computerized wherein borrowing medical records and returning it is no longer manual. The purpose of this information system is to facilitate officers in recording, borrowing, regulating, controlling, and regulating medical records. Borrowing and reversion medical records must be managed and controlled properly to reduce the loss of medical records and miss files. This study used a qualitative method with documentation, interviews, and brainstorming data collection taken at Baladhika Husada Hospital Jember. The method used in the research is the Waterfall model by Sommerville (2011). Stages that carried out are requirements definition, system and software design, implementation and unit testing, and integration and system testing. The design of this information system used flowchart designs, context diagrams, Data Flow Diagrams (DFD), and Entity Relationship Diagrams (ERD) and then implemented the design results into visual basic programming using the Microsoft Visual Studio 2010. The results of this study are information systems for borrowing and reversion medical records at Baladhika Husada Hospital Jember.
