Abstract
Completeness of the contents of the medical record according to the Indonesian Ministry of Health is said to be complete if the completeness of the contents reaches 100%. Based on the preliminary survey on 11 April 2018 there were 20 medical record files showed 80% of the initial assessment forms were not complete and 75% incomplete nursing and midwifery action documentation forms. The purpose of this study is to analyze the relation of knowledge, attitudes, superior's support and support of coworkers to the behavior of officers in completing the initial assessment form and documentation of nursing and midwifery actions at Citra Husada Hospital, Jember. The type of research is quantitative. Statistical tests using the spearman correlation test with < 0.05. The result showed that there was a significant relationship between knowledge (Sig 0.043), attitude (Sig 0.024) and coworker support (Sig 0.043), while supervisor support (Sig 0.413) there was no significant relation with the behavior of officers in completing the initial assessment form and documentation nursing and midwifery actions at Citra Husada Hospital Jember. The advice of this study is hospital need to hold training and seminars related to the importance of completeness of records on medical record forms.
