Abstract

Recording and Using ambulances are closely related to the report produced. A good record will generate better information. The problem occurred in Technical Service Unite of Politeknik Negeri Jember was the inconsistency in recording the data of ambulance usage, thus the information produced was also inaccurate. The purpose of this research was to design and create a web-based ambulance service application. The type of research used was qualitative study with the waterfall method. The first phase conducting waterfall method was requirement analysis and definition or analysis of system requirements to respondents. Second phase was continued to do system and software design by creating a flowcharts system, specifically DFD level 0 and 1, ERD, and implementing it into the programming language of PHP. The third waterfall stage was implementation and testing. Implementation was carried out on 2 users by a blackbox examination with 100% success (no errors). This ambulance application was able to help the Technical Service Unite of Politeknik Negeri Jember in managing the data of ambulance usage so that the report produced would be more accurate.
