Abstract

The Nahdlatul Ulama Banyuwangi Hospital has used the Hospital Management Information System (SIMRS), but the lending and returning of medical records is still manual. The Nahdlatul Ulama Hospital in Banyuwangi has organized medical records, but has several obstacles including missfiles and late return of medical record files. The purpose of this study is to create a system of borrowing information and returning medical record documents to overcome problems that occur, because they are still managed manually. The study was conducted by collecting data by observation, interview and brainstorming methods at the Nahdlatul Ulama Hospital in Banyuwangi. The development method used by the information system is the waterfall method with the stages of analysis and definition of requirements, system design and software, unit implementation and testing, as well as system integration and testing, for operation and maintenance are not examined. The design of this information system uses the design of Flowcharts, Context Diagrams, Data Flow Diagrams and Entity Relationship Diagrams. This information system uses the PHP programming language with a MySQL database for storage media. The results of this study are in the form of a loan information system and return of medical record documents that are useful to assist officers in controlling the management of medical records. The advantage of this research is that the report is presented in graphical form which can provide information that is easy to read and in the medical record return section, the record return status is created automatically ie if the officer who borrowed the medical record when returning exceeded the time limit specified in system, then the file is said to be late status.
