Abstract

Form of outpatient medical record contains the health services provided to outpatients. Medical records forms need a folder to protect in order not misfile. Forms of medical record should be designed based on user requirements. Based on the preliminary survey in Aminah Maternity Clinic Probolinggo Kraksaan, medical record forms of outpatient are not accordance with the provisions and user need, which causes misfile and incompleteness of form filling. Therefore, it needs redesigned to fit the needs and conditions of some aspects. The purpose of this study is to design form and folder of outpatient medical records in Aminah Maternity Clinic Kraksaan Probolinggo based on anatomical, physical and content aspect. This study use qualitative, it also uses observation and interview instrument, while the data analysis techniques using brainstorming. The result of this research is redesign of a new outpatient form and a new folder based on the anatomical aspects, adding the title of form, instructions, and introduction, also the content of form uses border 2.54 cm, while the folder include a title and medical record number. For physical aspect form of outpatient medical record uses a paper of HVS 80 gram in portrait form, the color is white and uses black ink. Folder uses a green manila paper. In the content aspect, there are item additions like phone number, date of visit, allergy history, ICD-X column, ICD9cm column and small boxes. This new result suggestion of form of outpatient medical record design is expected to be applied and evaluated by further research.
