Abstract

The admission and discharge summary’s form called summary or front sheet. Has always been in front at a medical record file. This form is one of the inpatient medical record form enshrined, is used as a summary to determine the patient's condition when entering until exit from hospital. Forms should be designed in appropriate with provisions to comply the needs of users. Based on the early survey at the Perkebunan Hospital (Jember Klinik) PT. Medika Nusantara Utama, design of admission and discharge summary’s form not in appropriate with the provisions and requirements of users resulting incompleteness charging that need their form control for further redesign existing form so that the form in appropriate with the provisions of all aspects. The purpose of this research to redesigning admission and discharge summary’s form based on the needs of users from the physical aspects, anatomic aspects and content aspect. This type of research is qualitative research with methods of data collection such as observation, interviews and Focus Group Discussion (FGD). The result of this research is the re-design the new admission and discharge summary’s form based on the physical aspect that the paper materials using HVS 80 gram. For anatomic aspect is the addition of title, instructions and the body be given the boundary lines of charging and using Times New Roman type style size 12 pt for all except the title font size be larger. As for the content aspect there are additional items the date and time admission, date and time discharge, identification of the patient's family, rooms, admission and discharge procedure. Suggestions for the design of admission and discharge summary’s form which is expected to be implemented and can be evaluated by further research.
