Abstract

The main of improvement system release of medical information is for keeping patient privacy or medical secrecy. Bhayangkara Bondowoso Hospital served the release of medical information to internal and external parties, but the problem is happening to the release of medical information not contained documentation and approval prior to the information provided to other parties. The purpose of this study is improving documentation of medical informationrelease and informed concent of medical release information with PDCA (Plan, Do, Check, Action) approachment. The type of this research is qualitative descriptive study by collecting data of observation, interviews and brainstorming methods. The results of this study is there a gap between Standart Operational Procedure (SOP) and plans that have been discussed by medical recorder. Bhayangkara Bondowoso Hospital related documentation and consent of medical release information. Patient it caused by lend and the demand consent of medical information through oral or verbal, only there fone, there is no recording or documentation of in and out medical record file, that lended for saking of medical release information. Patient it also impacts for occur misfile of medical records document. When it is needed and difficulty to see the current prior patient history. Therefore, the researcher designed the expeditionts form for documentation and consent form to demanel medical information patients to comply the Standart Operational Procedure (SOP) related lending medical records for other interests, as the anticipation interrelated of demands medical scret opening with out any permission of the patient or who have authority to release the patients medical information. From this a researcher suggest to do the result of improvement as the anticipation of guidance for medical secret opening to word unresponsible side.
