Abstract

Along with technological advances in various fields, all activities or human work to improve the quality of self is done with the development of computer technology is increasing rapidly from day to day. Information technology is one of the technologies created as a human aids to do various tasks so that the tasks done can be more effective, efficient, easy fast and accurate. When the registration of patients can not be separated from the application registration at the Puskesmas. Puskesmas Sumber Wringin less than maximum in carrying out patient registration because still using manual system. Then often the accumulation of patients in the waiting room due to the length of service at the registration counter. The purpose of this research is to make application of outpatient registration at Sumber Wringin Public Health Center by using waterfall method covering Requirement and Design, Encoding, Testing. This application only has access that is admin that is the responsibility of registration. In the application includes patient registration, patient data storage, patient data search, and generating patient visit information per day, per month and per year, and using automated voice calls for poly calls. Puskesmas Sumber Wringin Bondowoso is expected to further develop the application of outpatient registration for continuous information or integration of data needed in the development of outpatient registration process
