Abstract

Outpatient form of medical record is form that used to note all data of identity and data of clinical patient. Outpatient formthat used now is lack of data of identity and data of clinical patient that caused limited size and form of design. Form of the patient is just in page form because of nothing medical record map. This research aims to do map desaigned medical record of outpatient. The kind of this research is descriptive qualitative. Analysis of unit in this research is user of the medical record officer of TPPRJ, officer of medical record, doctor and nurse. Methodsfor collecting data in this reseach are observation, interview, and brainstorming. Based result of this research, map of medical record of the outpatient designed based user’s needed and Huffman standard and comes with a color code sticker, while the form of the medical record outpatient re-desaigned with added an important item to the identity of the data and clinical of the patient. Addition to form of medical record of outpatient is based on the physical aspect for the material HVS 80 gram, A4 size white. For anatomy aspect is added logo and coloumn or line limited filling. While for this aspect, these are addition of clock items, anamnesa, ICD code, history of allergy and name and Doctor’s signature.
