Abstract

Stroke is an acute neurological dysfunction caused by circulatory disorders of the brain , where the sudden in a matter of seconds or faster in a few hours the symptoms and signs in accordance with the focal area of the brain is interrupted . Based on statictical data of hospital, during 5 years (2011-2015), total of patients stroke was rise for life patients or dead patientsin RSD dr. Soebandi. The purpose of this study was to Identify causes of stroke based on medical record document period 2011-2015 in RSD dr. Soebandi. The type of this research is a descriptive study with cross sectional approach . The sample is medical record documents of patients stroke has passed away amount to 94 data. The process of aggregation of data used checklist. The result from this study is past stroke, age, and gender become highest cause stroke from unchangeable risk factors, while for changeable risk factors are hypertension, atherosclerosis, and hypercoesterollemia become the highest. Unit stroke factors weren’t causing mortality rate of stroke because appropriated with National Guideline Stroke, except availability of information source about preventive and curative stroke was exactly and quickly. Suggestion from this study is Hospitals are advised to do providing information to patient or family about preventive and curative about stoke exactly and quickly can be made directly or indirectly in the form of media such as advertisements , posters or flyers.
