Abstract
Hospital reporting is a healthcare service activity in hospitals tasked with collecting, processing, and presenting data into information. In a preliminary study at RSUD R.T. Notopuro Sidoarjo had an average of 14 reports out of 19 types or around 74.89% that were late each month. This research aimed to analyze the factors that caused delays in monthly reporting implementation using Lawrence Green's behavioral theory (1980), which consisted of predisposing factors (knowledge, attitudes, discipline and education), enabling factors (work facilities and training), and reinforcing factors (SOP and motivation). In this type of research, we used qualitative data collection methods, including interviews, observation, documentation, and brainstorming. The analysis revealed that poly section officers' lack of knowledge had caused delays in monthly reporting, resulting in late collection of reports. The attitude factor is that officers still put aside the implementation of monthly reporting and prioritize other work. The poly section officers' disciplinary factor was that they delayed the reporting implementation, as shown by the late delivery of data to the medical records section. The facility factor is that the SIMRS application cannot be accessed simultaneously from many related units. In the SOP, there was no clear time limit specified for submitting monthly reports. Medical records officers were expected to provide outreach about monthly reporting and remind relevant police officers if data collection faced delays in the past.
