Abstract

The rate of late return of outpatient medical records at the Ajung Jember Health Center in July 2022 - April 2023 was 5.36%, increasing to 20.07% in January-February 2024. Several factors are thought to be the cause of late medical record returns, including inappropriate educational qualifications of officers, the absence of rewards and punishments, suboptimal training, and ineffective implementation of the SOP for returning medical records. This study aims to analyze the factors causing the delay based on Mc Cormick and Tiffin's performance theory which includes individual factors and situational factors. This study used a qualitative method with research subjects including the head of the puskesmas, registration officers, poly admins, nurses, and midwives. The results showed that individual factors affecting delays were the educational background of officers and the application of rewards and punishments that were not optimal, while situational factors included missing training, lack of budget, lack of guidance and evaluation, implementation of SOP that were not optimal, and the use of ineffective expedition forms. Puskesmas Ajung is expected to conduct socialization to officers, optimize the application of punishment, training for all officers, submitting a budget for training, reviewing SOP, and adding a loan date column as monitoring material.
