Abstract

The current era of modernization, a good medical record must keep abreast of the time, one of the puskesmas in Jember that still applies medical records manually is Banjarsengon Puskesmas. As a result, there are obstacles in the process of services provided to patients such as the time for the provision of medical record documents on poly KIA is about 10 minutes to more, whereas in the Permenkes Republic of Indonesia No.129 in 2008 the standard time for providing medical record documents was less than 10 minutes for outpatient care. Not only that, the recording of medical records into one sheet on an outpatient form that includes 3 polyclinic causes officers often find difficulties to search for a patient's medical history and also often results in incomplete filling of maternal medical record forms and child medical record forms. One way to help the process of servicing to patients is by designing and making electronic medical records on poly KIA at Banjarsengon Health Center using Microsoft Visual Studio 2017 application and by the Waterfall method. From the research results obtained by the system design stages using Flowchart, Context Diagram (CD), Data Flow Diagram (DFD), dan Entity Relationship Diagram (ERD). The design of electronic medical records can facilitate officers in carrying out services and can maintain the confidentiality and security of medical record data. The advantage of this system is that it is more detailed, easily accessible, displays output in the form of a medical record form that is divided into a mother's medical record form and a child's medical record form, can display data on drug names in the Puskesmas, and displays the results of the visit and LB3 reports automatically according to the format in the Puskesmas and the Health Service.
