Abstract
The process of borrowing medical record files for the benefit of patient health services or outside the interests of care is recorded in the file lending book manually. This can be seen from the number of files that are late in returning plus the absence of a tracer as a substitute for the medical record files that come out. This study aims to create a loan and return information system that can generate QR codes, tracers and reports and can control the quality of the contents of medical record files using the watrerfall method. The waterfall method has several stages, namely needs analysis that produces the necessary data and a design description of the system, designing the system using flowcharts and data flow diagrams, coding the system with the Microsoft Visual Studio Code application and XAMPP in creating the database, testing the system using a black box, then implemented. The final result of this research is a repayment loan information system and the completeness of outpatient medical record files that simplify the process of controlling loans and quality as well as the quantity of medical record files.
