Abstract
The standard time for borrowing and returning outpatient medical records is 24 hours. However, a problem has arisen at Wirasakti Jember Clinic where there are delays in returning medical records that exceed 24 hours due to the manual recording process for borrowing and returning. Therefore, a system is needed to address this issue by creating a medical record borrowing and return information system. The research method used in this study is research and development, with data collection through interviews, observations, brainstorming, and documentation. The waterfall method by Rossa and Salahudin is employed, consisting of analysis, design, coding, and testing stages. The analysis stage involves problem analysis and system requirements. The design stage produces flowchart, DFD, and ERD. The coding stage employs the programming languages PHP, HTML, Bootstrap, CodeIgniter 3, and MySQL. The testing stage uses black box testing to evaluate the functionality of the system produced. This research results in a web based medical record borrowing and returning information system at Wirasakti Jember Clinic, with features such as tracer printing, system and email notifications, and completeness checks of medical records.
