Abstract

The goal of the research is to designing forms in general outpatient Patrang community health center based on aspects of content, aspects of Anatomy, and the physical aspect. This type of research is qualitative. Data capture techniques used observation, interviews, and brainstorming. The results of this research is the design of a form in the general outpatient Patrang community health center year 2018 corresponding to standard Kemenkes number 269 years 2008 about medical record and Huffman (1999). The results of this research is the design of the outpatient medical record form poly common in Patrang community health center based on anatomic aspect i.e. the used margin-left 2 cm, right-2 cm, bottom-2 cm, 1 cm above and add items form filling instructions, identification number, form, and item close date, i.e., the name and signature of the physician. For the physical aspect in the form of paper F4 70 grams of hvs-shaped inky black and potrait. In addition there is the aspect of the content of the medical record, the addition of code word files secret, and small boxes to make it look more presentable. Suggestion form design results outpatient medical rec poly new General is expected to be implemented and evaluated by the researchers.
