Abstract
The destruction of medical record documents at RSUD dr. R. Soedarsono Pasuruan has not been conducted. It is because there is no supporting application to transfer the medical record forms that should not be destroyed such as discharge summary forms, resumes, surgery sheets, baby identification, approval sheets, death sheets, and another unique case occurs to the patients. The purpose of this research is to design and make a web-based media transfer application to support the retention process and the destruction of medical record documents. The developing waterfall system method was used in this study. It is a system that consists of requirements definition to produce the information system requirements, system design and software to produce the designs in the form of flowcharts, context diagrams, DFD, and ERD. The implementation step and unit testing produce parts of the program which are combined and tested at the integration and testing system. The research result is a media transfer application that support the retention process and the destruction of the medical record documents that have been tested properly and implemented in RSUD dr. R. Soedarsono Pasuruan.
