Abstract

The completeness and neatness of Childbirth and Partograph Observation Forms are keys to becoming informative medical records in orderly administration. All of those things have not been done well at the Suherman Clinic, caused by wasteful use of paper, overlapping data items, and the implementation of unfavorable filling procedures. The purpose of this research is to design Childbirth Observation Forms and Partograph in accordance with the theory, standards, and user needs using software Ms.Office Word 2010. The type of research is descriptive qualitative using observation, interview methods, and brainstorming. The validity test using source triangulation and technical triangulation. This research began in June to December 2018. The subjects of research used purposive sampling consisting 4 respondents. Variables consisted of the physical aspects, anatomical aspects, and aspects of the contents. The results of this research are made uniformity of fonts and materials for each form; then delete some items on the physical inspection line, some additions such as Heading and Close; addition of option circle frames; new design phase I observation table; partograph filling index frame; lane box 13-16 hours; additional items in recording phase I, and additional information on newborns. The new form has been applied well in the clinic, according to user requirements and standards. The results of filling evaluation in the new form show 100% complete. The researcher suggested that design such this research could be design by using another software and continuously evaluated, also adjusted to the needs of the user and the applicable standards.
