Abstract
Based on the preliminary study, there were 10 patient medical record files that duplicated in the July 2018 to September 2018. There was a problem about duplication of medical record numbers in the Yosowilangun Public Health Center. The purpose of this study is to analyze the factors that cause the duplication of medical record file numbering in the Yosowilangun Public Health Center using Tree Diagram method. The results of this study showed the factors that cause duplication of medical record numbers were the Man factor (registration officer still has other works outside of the specified job description, the registration officer did not fulfil the education minimum standart of medical record yet), Money (There did no budget for making applications that can minimize the duplication of medical record numbers), Material (did not have KIUP), Machine (did not have an application to minimize the duplication of medical record numbers), and Method (It was not factor causing duplicate medical record numbers). Suggestions for this study are the Chief of Yosowilangun Public Health Center proposes adding medical record officers to the Health Service, providing training on medical records to registration officers and making application systems that can minimize duplication of medical record numbering.
