Abstract

The Nguling Primary Healthcare Center has never conducted retention and extermination of MR where in 2021 this place has 55,861 MR that have not been retained which causes a buildup of files on the MR storage shelves. The purpose of this study was to analyze the causes of the non-implementation of MR retention and extermination at Nguling Primary Healthcare Center. This type of research is qualitative. Collecting data using interviews, observation, documentation. Informants in this study consisted of the Head of the Nguling Primary Healthcare Center, the head of medical records, 2 medical record officers who were taken using a purposive sampling technique. The results of the study based on the 5M indicators (man, method, money, materials, machine) show that the cause of the non-implementation of the retention and extermination of MR is man factor that is most of the officers do not have an RMIK educational background, the lack of knowledge of officers regarding the retention and extermination of MR, and the officers have not followed MR retention and extermination special training. Method factor is that there is no SOP of MR retention and extermination . Money factor is that there is no budget for MR retention and extermination activities. The causes for the non-implementation of MR destruction include material factors is active and inactive MRs have not been separated, there is no list of descriptions and machine factors, namely there is no scanning tool and no file destruction tool. The reason why MR retention is not implemented is machine factor which includes no shelves and MR storage space. Results of the priority problems with CARL method are that training has not been carried out, there is no budget and there is no SOP on MR retention and extermination . Solution obtained based on the results of brainstorming was socialization by skilled medical record officers, submitting a budget for retention and extermination of MRs and making SOPs for retention and extermination of MRS.
