Abstract

Shrinkage and destruction had not yet been carried out at the Lenteng Health Center, so medical records were found piled up and not placed on storage shelves. This study aims to analyze the causes of the depreciation and destruction of inactive medical record files at the Lenteng Health Center based on the 6M management element. This type of research is qualitative research. Prioritizing problems using USG (Urgency, Seriousness, Growth) and solving recommendations using brainstorming. The subjects in this study were 1 puskesmas head, 1 UKP (Individual Health Effort) coordinator and 3 medical record officers. Collecting data using in-depth interviews, observation, brainstorming and documentation. The research instrument used interview guidelines, observation guidelines, documentation guidelines, ultrasound guidelines and brainstorming guidelines. The validity of the data using technique and source triagulation. Data analysis uses data collection, data reduction, data display, and conclusion drawing. The results of the study are based on the 6M management elements, namely man (educational qualifications are not up to standard and have no effect on educational variables, medical record officers have never attended training), method (SOP (Standard operational procedure) depreciation and destruction into one and not socialized), money ( unavailability of a special budget for depreciation and destruction), materials (no shelves and storage space for inactive medical record files, no description forms, destruction minutes, assessment forms and retention schedules), machines (no scanners and enumerators), motivation (not a factor causing depreciation and destruction). The main priority issue is the element of the method, namely the SOP for depreciation and destruction into one with storage and not socialized. The research suggestion is that medical record officers are expected to implement the SOP for depreciation and destruction recommended by the researcher.
