Abstract

The standard time for borrowing and returning medical record is 1/24 hours. The problem at Ajung Jember Health Center is the delay in returning medical record files because the recording of borrowing and returning medical record files is still manual. The general purpose of this research is to design and create a web-based information system for borrowing and returning medical records at Ajung Health Center. This research is a qualitative research, namely to collect data using interviews, observations, and documentation. The method used by the researcher is the Waterfall method by Sommerville. The stages of the Waterfall method are requirements analysis and definition, system and software design, implementation and unit testing, integration and system testing, operation and maintenance. The researcher did not do the operation and maintenance stage. The result of the requirements analysis and definition stage is the officer wants a notification of the return of medical record and tracer files. The results of the system and software design stage are system flowcharts, context diagrams, DFD and ERD. The result of the implementation and unit testing stage is a web system for borrowing and returning medical records. The result of the integration and system testing stage is that the officer can operate the information system for borrowing and returning medical records properly. The advantage of this research is there are notifications and tracers to help officers to control the borrowing of medical records. The lack of this research is that the information system for borrowing and returning medical records is not integrated with the existing system at the Ajung Health Center and the tracer is not yet in the form of a barcode. The conclusion of this research is the problem of borrowing and returning medical records at Ajung Jember Health Center can be overcome by designing and creating a web-based information system for borrowing and returning medical records.
